AI adoption is accelerating rapidly however, the value from AI isn’t being realised as quickly.

Research shows that while the majority of organisations are now using AI, only a small proportion are achieving meaningful business impact.

At the same time, over half report negative consequences from AI use, including inaccuracy, risk exposure and unintended outcomes.

This is a human and organisational systems problem and a leadership challenge.

The Risk organisations are underestimating:

AI does not remove bias, instead, it learns from and scales what already exists. Evidence shows that AI systems can:

  • Reinforce bias (including gender, racial, age bias) present in historical data
  • Influence human decision-making in ways that amplify discrimination, rather than reduce it, and
  • Produce outputs that appear neutral, but actually embed structural inequities

For example, studies of generative AI tools show systemic gender and racial bias in outputs, often reinforcing stereotypes beyond real-world baselines. Research into recruitment decisions shows that when AI is used, humans tend to follow its recommendations, even when they recognise a bias has played out. Real world applications are already showing impact, including AI systems in public services that downplay women’s needs compared to men’s.

Without intervention, AI does not just reflect inequity, it amplifies it at speed and at scale. Organisations that succeed with AI will not be those that move fastest, they will be those that build the right foundations first in order to make the right impact.
